ClinicalConnectTM is a secure web portal delivering an integrated Electronic Health Record (EHR) to thousands of Physicians and Healthcare professionals across the Hamilton Niagara Haldimand Brant (HNHB) and Waterloo Wellington (WWLHIN) LHINs. Whether on computers or mobile devices, providers can view real-time consolidated information, in seconds, from 31 regional hospitals, CCACs and Oncology for patients in their Circle of Care resulting in quicker diagnosis, treatment and improved patient care and safety.
ClinicalConnectTM can reduce the need for repeat lab and diagnostic tests and the time searching or sending patient information. Patient medical information is integrated, whether the patient was admitted to hospital in Hamilton, received a diagnostic test in Kitchener, or is receiving access to services from the CCAC. Physicians can also electronically transfer lab results and reports securely, via the integrated Hospital Results Manager process into their office Electronic Medical Record. Mobile access means providers can be better connected to their patient’s ongoing care.
