Home and Community Care staff can include a care note in a client’s record. If a note has been shared by Home and Community Care it will be available in ClinicalConnect. This tab contains any information available from the acCDR if the patient has HCC services from beyond south west Ontario. The Home/Community Care column will display the name of regional HCC.
Viewing Note Details
To view the note details, select click the note from the list.
Depending on your preference settings, the Note Detail window will either show up to the right of the orders list or below the orders list.
Click the Close button to close the Note Detail screen.
Note Details for HCC Services from Beyond South West Ontario (acCDR data)
The List view displays a subset of data from the acCDR in this module. To display additional Note details available from this provincial repository, click the line item to view the detail window.
Viewing Note Details
The Notes tab has its own set of preferences that can be accessed and configured by clicking the Preferences button 
in the header or by using the Preference Setup Wizard.
After updating your preference settings, click the Save & Apply button to save your changes.
