The Referral List displays the patient’s referrals based on the status of Pending, Admitted or Discharged referrals.
This tab will contain any information available from the acCDR if the patient has HCC services from beyond south west Ontario. The Home/Community Care column will display the name of LHIN HCC.
Click the Referral tab (if available) to access the Referral List.
The following table provides a summary of some of the key features of the Risks tab.

As defined by Home and Community Care. Examples: Acute Maintenance, Rehab, Short Term Respite. |

As defined by Home and Community Care. Status can includes Active, Pending, or Discharged. |
Referral List Preferences
The Referrals tab has its own set of preferences that can be accessed and configured by clicking the Preferences button 
in the header or by using the Preference Setup Wizard.
After updating your preference settings, click the Save & Apply button to save your changes.
