The Long Term Care (LTC) Choices and Bookings tab displays all long-term care placement referrals for the client as well as current waitlist placements for long-term care based on the client’s preferred selection of LTC facilities. 

Click the LTCH Choices and Bookings tab (if available) to access LTCH Choices and Bookings.

The following table provides a summary of some of the key features of the LTCH Choices and Bookings tab.

Referral Type

Example of Referral Types include:

  • Home Care
  • School
  • Other Reimbursement Programs
  • Long Term Placement
  • Short Stay – Interim
  • Short Stay – Convalescent Care
  • Short Stay Respite
  • Complex Care Behavioural Health
  • Complex Care End Of Life
  • Complex Care Medically Complex
  • Complex Care Restorative
Rank

Indicates ranking as identified by the client and family.

Priority Name

Based on the Home and Community Care needs assessment. Entry can include:

  • 1 – Crisis - People who need immediate admission to long-term care and cannot have their needs met at home, or who are in hospital, when hospital is in crisis. People in long-term care home that is closing within 12 weeks.
  • 2  – Spouse/Partner Reunification – Transition from a Specialized Unit People who need to be reunified with their spouses/partners who are currently residing in a long-term care home, and who meet eligibility requirements (including care needs).
  • 3A - 4A + Ethno-Cultural/Religious choice - People waiting for a long-term care home serving those of a particular religion, ethnic origin or culture. People who have high care needs, but can still be supported at home until a bed becomes available. People in hospital waiting for long-term care. People in a long-term care home seeking transfer to their home of choice.
  • 3B – 4B + Ethno-Cultural/Religious choice - People waiting for a long-term care home serving those of a particular religion, ethnic origin or culture. People with care needs who are currently managing at home with supports. Wait times for clients in this category are much longer.
  • 4A – Comm High Needs/Hosp ALC/LTCH transfer to 1st choice - People who have high care needs, but can still be supported at home until a bed becomes available. People in hospital waiting for long-term care. People in a long-term care home seeking transfer to their home of choice.
  • 4B – Community/LTCH transfer to secondary choice - People with care needs who are currently managing at home with supports. Wait times for people in this category are much longer.

 

Viewing LTCH Choices and Bookings Details 

Placements are displayed in a sortable list. Click the row of the note you want to view to see more details about the placement.

Depending on your preference settings, the Placement Detail window will either show up to the right of the LTCH Choices and Bookings list or below the LTCH Choices and Bookings list.

Click the Close button to close the Placement Detail screen.

 

LTCH Choices and Bookings Preferences

The LTCH Choices and Bookings tab has its own set of preferences that can be accessed and configured by clicking the Preferences button Image of the preferences button in the header or by using the Preference Setup Wizard.

After updating your preference settings, click the Save & Apply button to save your changes.
