Resource Centre - Home and Community Care Module Overview
The Home & Community Care (HCC) module includes data made available from the Client Health Related Information System (CHRIS) from the following south west Ontario Local Health Integration Networks (LHINs):
- Hamilton Niagara Haldimand Brant (HNHB)
- Waterloo Wellington (WW)
- Erie St.Clair (ESC)
- South West (SW)
This module also contains data from Home and Community Care Services from beyond south west Ontario LHINs, made available from the Acute and Community Clinical Data Repository (acCDR). ClinicalConnect™ users who currently have access to acCDR data through ClinicalConnect™ will be enabled to access the additional Home and Community Care data available through this integration enhancement.
The HCC module displays information in tabs where information is available from home and community care providers. If there is no information from home and community care providers for a specific tab, that tab will not display in the Home and Community Care module. Tabs identified as Active include information that is current. Historical data is not available.
* Patient records with Home and Community Care information available in acCDR, will now display in ClinicalConnect in the select tabs as shown in blue below.
To access the Home and Community Care module, click the Home/Community Care option in the Clinical Modules menu.
Patient Search Results displaying Home & Community Care Encounters
When searching for a patient in ClinicalConnect, the search results will include any Home and Community Care encounters. HCC encounters are identified with the specific LHIN-based Home and Community Care in the facility column and OACCAC in the source field. If the client has had HCC services across all four regions, each encounter will be identified in the facility field.
If the patient has HCC services from beyond south west Ontario and if there is data available in the acCDR. The facility will display the name of LHIN HCC and source column will display eHealth Ontario.
Selecting the Home and Communty Care encounter from the search results list will open the Home and Community Care module for that patient.
Acute and Community Clinical Data Repository (acCDR) Consent Directive
A patient has the right to withdraw consent to the access and use of their full or partial records from acCDR through the current eHealth Ontario process. ClinicalConnect honours any partial or full consent restriction and will display the following message in the Transcription and Visits modules:
There is a consent directive in place for the selected patient.

Changing Your Default Home and Community Care Tab
You can set which tab to open first when you access the Home and Community care module. To set your default Home and Community Care tab:
1. Click your user name in the top right corner of the screen to access the user menu.
2. Click Global Preferences.
3. Click the Collapse All button.
4. Click the Home/Community Care (click to expand/collapse) option.
5. Set the Default View dropdown to the tab you’d like opened first each time you access Home and Community Care.
6. Click the Save button.
